MacPractice and Submit Patient Forms Collaborate

Lincoln, NE, August 5, 2009 - MacPractice, Inc., the developer of MacPractice MD, MacPractice DDS, MacPractice DC, and MacPractice 20/20, the leading practice management and clinical applications for Mac, announces an agreement with Submit Patient Forms of Malverne, NY to offer a low-cost solution for medical, dental and chiropractic offices for their patients to complete their forms on the Internet, prior to their arrival in the office.

For a monthly fee of $49 or $499 annually (a savings of 15%), a practice may choose to make a variety of standard forms available online to each individual patient, including registration, HIPAA, insurance authorization, medical release, and medical history. Standard forms may be modified and new forms added for a one-time fee of $995 for custom forms up to 12 pages.

Once completed, patient forms may be viewed online, printed or downloaded to the office computer as a pdf or jpeg, tiff to be associated with the patient record utilizing MacPractice Attachments. The data on the form can also be extracted as a text file for use in another program, and potentially in MacPractice MD, DDS, DC and 20/20 in future versions.
